Call for Nominations


The CAG is pleased to issue a Call for Nominations for the Queen Elizabeth II Diamond Jubilee Medal.  We encourage our members to submit nominations to us for consideration.  Nominations submitted to the CAG will be reviewed by a CAG selection committee, and nominations will be forwarded to The Chancellery of Honours, Office of the Secretary to the Governor General.  Medals will be presented at the 2012 Annual Scientific and Educational Meeting of the CAG, “Aging in a Changing World,” October 18 – 20, 2012 in Vancouver, BC.[1]
 
The Queen Elizabeth II Diamond Jubilee Medal

A new commemorative medal was created to mark the 2012 celebrations of the 60th anniversary of Her Majesty Queen Elizabeth II’s accession to the Throne as Queen of Canada. The Queen Elizabeth II Diamond Jubilee Medal is a tangible way for Canada to honour Her Majesty for her service to this country. At the same time, it serves to honour significant contributions and achievements by Canadians.  During the year of celebrations, 60 000 deserving Canadians will be recognized.
 
Eligibility Criteria
 
To be eligible for this honour, a person must:
  • Be a Canadian citizen or a permanent resident of Canada, but need not necessarily reside in Canada;
  • Have made a significant contribution to a particular province, territory, region or community within Canada, or an achievement abroad that brings credit to Canada; and
  • Be alive on February 6th, 2012, the 60th anniversary of Her Majesty’s accession to the Throne. The medal can be awarded posthumously, as long as the recipient was alive on that date.
Guidelines for the Selection of Recipients
 
The following guidelines have been developed to provide guidance in selecting deserving candidates:
  • The awarding of the medal should focus on the achievements of persons who have helped to build the Canada of today, including seniors, volunteers, veterans and youth;
  • Through their achievements and sustained contribution, the candidates have distinguished themselves from othersemployed in the same field;
  • Recipients should be highly respected individuals within their community. The recipients’ names will be part of the public record;
  • The list of recipients should be inclusive and reflect the demographics of the region or community;
  • The selection process should be non-partisan and merit-based;
  • The medal is to be awarded to individuals only, not to groups or couples
To submit a nomination, please complete the nomination form.  You will be asked to provide your contact information; contact information for the nominee; and a short description (40 words/250 characters) of the candidate’s achievements.  Nominations may be submitted in English or French.
 
Nominations are due by September 6, 2012.  Only nominations submitted through the online form will be accepted; please do not submit nominations by email.
